Tender Touch Rehab Services LLC in Hackensack, NJ is seeking a licensed Physical Therapist to join as a Full-Time Lead. You will evaluate and treat patients in skilled nursing facilities, implement personalized care plans, and collaborate with an interdisciplinary team.

Qualifications

• Degree from an APTA-accredited Physical Therapy program (BS, MS, or DPT).
• Current license as Physical Therapist in the state of practice.
• Strong communication and organizational skills.
• A solid year of SNF experience and willing to travel to multiple facilities.

Responsabilités

• Evaluate and treat patients with mobility, strength, balance, and functional impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all required physician orders are obtained and up to date.
