Job summary

Tender Touch Rehab Services, part of Enhance Therapies, is seeking a dedicated Physical Therapist for a full-time role in a skilled nursing facility setting in Connecticut. You will evaluate and treat patients, develop personalized care plans, and collaborate with a multi-disciplinary team to achieve optimal outcomes.

Qualifications

• Degree from an APTA-accredited Physical Therapy program (BS, MS, or DPT).

Responsibilities

• Evaluate and treat patients with mobility, strength, balance, and functional impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all required physician orders are obtained and up to date.
