Job summary

Renewal Rehab, part of the Enhance Therapies family, is hiring a Physical Therapist - FT to join our growing team at a skilled nursing facility in Illinois. You will evaluate patients and develop personalized plans while collaborating with care teams to achieve strong outcomes.

Qualifications

• Licensed to practice Physical Therapy in the state of practice.
• Degree from an APTA-accredited PT program (BS, MS, or DPT).
• Excellent communication and organizational skills.

Responsibilities

• Evaluate and treat patients with mobility, strength, balance, and functional impairments.
• Create and implement personalized care plans.
• Collaborate with team to ensure quality outcomes.
• Maintain documentation meeting regulatory requirements.
• Ensure physician orders are obtained and current.
