You will evaluate and treat patients with mobility, strength, balance, and functional impairments, and create personalized care plans while collaborating with the care team to ensure quality outcomes.

• Create and implement personalized care plans for each patient.
• Collaborate with the care team to achieve quality outcomes.
• Maintain documentation to meet regulatory requirements.
• Obtain and verify all required physician orders and updates.
