Qualifications

• Licensed Physical Therapist in Massachusetts.
• Home health or outpatient experience preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and manage a patient caseload.
• Excellent organizational and time management abilities.

Responsibilities

• Conduct comprehensive PT evaluations and develop individualized treatment plans.
• Improve strength, mobility, balance, and functional independence through interventions.
• Help patients safely achieve mobility goals in their home environment.
• Educate patients and caregivers on exercises and fall prevention.
• Collaborate with families, physicians, and team members to coordinate care.
• Monitor progress and adjust treatment plans as needed.
• Complete timely clinical documentation and maintain regulatory compliance.
• Maintain current clinical knowledge through continuing education.
