Madison Pointe Care Center in New Port Richey, FL is seeking a licensed Physical Therapist to join our rehabilitation team. You will evaluate patients, develop individualized plans, and deliver evidence-based interventions to improve function and quality of life for both short-term and long-term residents.

The ideal candidate collaborates with nursing, OT, SLP, and physicians, maintains thorough documentation, and supports care conferences and quality initiatives.

Qualifications

• Current Florida Physical Therapist license in good standing.
• Experience in a skilled nursing facility, post-acute rehab, or long-term care setting preferred.
• Strong communication, organizational, and interdisciplinary teamwork skills.

Responsibilities

• Provide comprehensive PT evaluations and individualized treatment plans.
• Deliver evidence-based interventions to maximize functional independence.
• Demonstrate knowledge of PDPM, Medicare guidelines, and documentation practices.
• Collaborate with OT, SLP, nursing, physicians, and other team members to coordinate care.
• Maintain accurate, timely documentation of medical necessity and progress.
• Participate in care conferences, discharge planning, and quality improvement initiatives.
• Monitor progress and adjust treatment plans to achieve measurable outcomes.
• Communicate effectively with residents, families, and facility leadership.
