Qualifications

• Must be a graduate of an accredited Physical Therapy program.
• Current Ohio Physical Therapist license or eligibility for licensure.
• Strong clinical judgment, communication, and documentation skills.
• Compassionate, dependable, and resident-focused approach to care.
• Ability to work independently while collaborating with an interdisciplinary team.

Responsibilities

• Evaluate residents to assess mobility, strength, balance, and functional limitations.
• Develop and implement individualized, evidence-based physical therapy treatment plans.
• Provide skilled physical therapy services to improve mobility, safety, and functional independence.
• Monitor resident progress and adjust treatment plans and goals as appropriate.
• Collaborate with occupational therapists, speech-language pathologists, nursing staff, and other healthcare professionals.
• Educate residents, families, and caregivers on safe mobility, fall prevention, and rehabilitation strategies.
• Maintain accurate and timely clinical documentation in the EMR.
• Follow applicable Medicare, Medicaid, and skilled nursing facility regulations.
• Deliver compassionate, resident-centered care that supports meaningful rehabilitation outcomes.
