Job summary

Wallace Rehabilitation And Healthcare Center in Wallace, NC is seeking a part-time Physical Therapist to join our long-term care team. You will perform comprehensive PT evaluations, develop treatment plans, and collaborate with the interdisciplinary team to improve resident outcomes.

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.
• Holds current license/registration in the state of North Carolina.

Responsibilities

• Provide comprehensive PT evaluations based on physician orders.
• Document findings on standardized forms and adhere to guidelines.
• Develop comprehensive treatment plans with goals, frequency, duration, and modalities.
• Obtain MD orders for evaluations, treatments, re-certifications, and discharges.
• Deliver treatments using appropriate modalities and adjust as needed.
• Document daily participation and weekly progress.
