Windsor Rehabilitation And Healthcare Center in Windsor, NC is seeking a dedicated Physical Therapist for either full-time or part-time roles. You will perform comprehensive PT evaluations, develop and document treatment plans, and ensure MD orders are obtained for all procedures and re-certifications.

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.
• Holds current license and/or registration in the state (as applicable)

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD’s orders.
• Document findings on the standardized evaluation form, accurately and adhering to guidelines.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities.
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modify treatment as indicated while adhering to precautions.
• Document patient’s daily participation and progress on weekly progress note accurately and timely
