Job summary

Ramseur Rehabilitation and Healthcare Center in Ramseur, NC is seeking a full-time Physical Therapist to join our Long Term Care facility team. We offer competitive wages, a flexible schedule, and a supportive work environment.

Responsibilities include evaluating patients per MD orders, developing treatment plans, documenting progress, coordinating with the interdisciplinary team, and providing education and supervision as needed.

Qualifications

• Graduate of an accredited university with a PT degree recognized by the APTA
• Holds current license and/or registration in the state where practicing

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD orders.
• Document findings on standardized evaluation forms with accuracy and compliance.
• Develop a comprehensive treatment plan with long- and short-term goals, frequency, duration, and modalities.
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Provide treatment to patients using modalities and adjust plans as needed while following precautions.
• Document daily participation and progress on weekly progress notes in a timely manner.
• Update treatment plans and discharge summaries as indicated.
• Complete facility documentation and ensure MDS-related information is accurate.
• Attend meetings and participate in interdisciplinary team discussions.
• Schedule patients as needed and perform screens.
• Bill patients ethically and accurately for PT services rendered.
• Cover colleagues’ caseload during absences and supervise junior staff or students.
• Maintain professional relationships with staff, residents, and families.
• Provide in-service education on patient care or PT services as required.
• Comply with facility policies and patient confidentiality rules.

Education

PT degree (B.S., M.S., DPT) recognized by APTA

Current license/registration in state
