Job summary

West Haven Center for Nursing and Rehabilitation is seeking a dedicated physical therapist to assess, plan, and implement therapy programs for diverse patient needs. You will develop personalized care plans, employ evidence-based treatments, and collaborate with a multidisciplinary team to improve functional outcomes.

Qualifications

• Must hold a graduate degree in physical therapy and a valid license.
• Experience as a physical therapist with diverse patient populations is preferred.
• Strong interpersonal and communication skills are essential.

Responsibilities

• Identify and meet patients’ goals and needs
• Reduce the need for medications and provide alternatives to surgery
• Develop care plans using a variety of treatment techniques
• Create fitness- and wellness-oriented programs tailored to patients’ specific needs
• Provide quality, personalized and evidence-based care and proven interventions
• Motivate patients during treatment to help them function optimally
• Promote healthy lifestyle by improving strength, flexibility, balance and coordination
• Consult and practice with other health professionals
