Job summary

Innovate Rehab and Wellness in St. Louis, MO is hiring a Licensed Physical Therapist (PT) for a full-time role in a Skilled Nursing Facility. Work Monday through Friday with flexible scheduling at a competitive hourly rate based on experience.

You will evaluate patients, create individualized treatment plans, and collaborate with an interdisciplinary team to restore movement, function, and quality of life while focusing on outcomes and patient satisfaction.

Qualifications

• Doctorate or Master’s in Physical Therapy from an accredited program.
• Valid state PT license.
• Clinical experience preferred; new grads welcome.
• Strong communication, problem-solving, and patient-care skills.
• CPR certification preferred.

Responsibilities

• Perform comprehensive PT evaluations.
• Develop personalized, evidence-based treatment plans.
• Provide hands-on therapy: manual techniques, therapeutic exercise, mobility training.
• Educate patients and caregivers on home programs and injury prevention.
• Track and document progress; adjust plans as needed.
• Collaborate with physicians, nurses, OTs, and other clinicians.
• Stay current with PT best practices through continuing education.
• Ensure compliance with regulatory and ethical standards.
