Responsibilities

  • Performs initial and ongoing physical therapy evaluations.
  • Provides physical therapy services as ordered by the physician.
  • Develops care plans with short and long-term goals with patient and caregiver involvement.
  • Performs clinical tests to identify functional limitations and restoration potential.
  • Monitors patient progress and reports responses to treatment to physicians and nurses.
  • Fits, trains, and inspects assistive devices for self-care and ambulation.
  • Participates in evaluation of position performance and performance improvement.

Job description

Performs initial and ongoing physical therapy evaluation according to APTA standards of practice and New York State Law and Rehabilitation Department guidelines.

  • Providesphysical therapy services for the patient, as ordered by the physician.
  • Develops a plan of care for the patient, in collaboration with the nurse, identifyingshort and long termgoals and patient and caregiver involvement.
  • Performs clinical tests to discover functional limitations and the potential for restoration and develops a plan of care with restorative and maintenance goals to meet identified needs.
  • Observes, records and reports to the physician and the nurse, the patient’sreaction to treatment and any changes in thepatient'scondition.
  • Works with the health team to shorten the patient recovery from illness and incorporates the management of residual disability.
  • Fits assistive devices forself-careand ambulation; trains patient in the use of devices and inspects for function.

Compensation:
  • $140 SOC, $80 per visit, $105 per Eval and $112.50 per Recert
