Job summary

River Valley Home Health & Hospice seeks a licensed physical therapist to provide in-home physical therapy services under a physician’s plan of care. You will assess patients in their home environment, develop and adjust the plan of care, and educate patients and families on therapy and prosthetic devices.

The role involves coordinating with the care team, documenting progress, and contributing to discharge planning. Two years of home health experience and valid state licensure are required.

Qualifications

• Degree from a baccalaureate or master’s program in physical therapy approved by an accredited organization.
• Licensed to practice as a physical therapist within the State, if applicable.
• Two years of appropriate experience as a physical therapist, with community/home health experience preferred.
• Good verbal and written communication and organization skills.
• Current CPR certification.
• Licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.

Responsibilities

• Provide physical therapy services to patients according to a written physician’s plan of care.
• Assess and evaluate therapeutic, rehabilitative, and functional status, and participate in the development of the total plan of care.
• Evaluate the patient’s home environment.
• Assess muscle strength, mobility, gait, range of motion, and potential for rehabilitation.
• Direct physical therapy treatment and instruct patients and families/caregivers in the use and care of therapeutic appliances.
• Determine priority needs for physical therapy and report to physician the patient’s reaction to treatment or changes in condition.
• Initiate physical therapy programs and instruct other personnel and/or family/caregiver members in certain phases of therapy.
• Train patients in the use of prosthetic devices when appropriate.
• Identify patient and family/caregiver needs for other home health services and refer as necessary.
• Prepare and submit clinical and progress summaries based on the attainment of goals.
• Participate in discharge planning for patients.
• Provide physical therapy consultation to home families/caregivers when indicated.
• Provide in-service education programs for nursing organization personnel as needed.
• Participate in peer consultation process.
• Supervise Physical Therapy Assistants according to organization policy and state regulations.
• When therapy is the only skilled service, instruct, supervise, and evaluate home health aide care every two (2) weeks.

Job Summary
