Qualifications

• Bachelor's, Master's, or Doctorate in physical therapy from an accredited program.
• Certification and licensure requirements as established by state and national board associations.
• Active state license required.
• Excellent organizational and time management skills.
• Excellent written and verbal communication skills.
• Possess current working knowledge of Functional Pathways policies and procedures.
• Ability to supervise Physical Therapy Assistants.

Responsibilities

• Screen residents quarterly to identify declines in functional abilities.
• Evaluate residents by MD order and establish appropriate treatment plans.
• Reassess patients every 30 days to determine need for continued services.
• Supervise treatment implementation by Licensed Physical Therapy Assistants.
• Provide thorough discharge planning to maximize patient independence.
• Educate staff, family, and caregivers as needed.
