Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide hospice therapy in patients' homes in San Antonio, FL. This PRN role offers flexible assignments, clinical autonomy, and the chance to support older adults in maintaining independence and quality of life.

Applicants must hold an active Florida PT license and a degree in Physical Therapy from an accredited program; new graduates are welcome.

Qualifications

• Licensed Physical Therapist in Florida with active license.
• Degree in Physical Therapy from an accredited program (BPT/MSPT/DPT).

Responsibilities

• Evaluate and treat residents in an assisted living community and/or home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
