Job summary

Fox Rehabilitation is seeking a licensed Physical Therapist to deliver one-on-one geriatric care in Delran, NJ, and surrounding areas. The role supports patients in private residences and senior living communities, promoting mobility and independence through our Geriatric House Calls model.

Clinicians may incorporate telehealth and virtual care to extend access and flexibility, while maintaining autonomy to build a personalized schedule and strong patient relationships.

Qualifications

• Licensed Physical Therapist in the state(s) of practice or eligible to obtain licensure.
• Current CPR certification.
• Basic computer/EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations and develop individualized plans of care.
• Deliver evidence-based therapy sessions to improve mobility and independence.
• Document patient care in the EMR per regulatory standards.
• Collaborate with interdisciplinary team including PT/OT/SLP and Fitness Specialists.
• Educate patients and caregivers to promote safety and aging in place.

Responsibilities

• Provide one-on-onephysical therapyto older adults wherever they call home (Privateresidences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
• Perform evaluations, develop individualized plans of care, and deliverevidence-basedtreatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR systemin accordance withregulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care whenappropriate toextend access and flexibility

Qualifications

Physical Requirements

FOX clinicians must be able to meet the physical demands ofin-homeandcommunity-basedcare, including:

• Exerting medium physical effort, including lifting 25–50lbsoccasionally, 10–25lbsfrequently, and up to 10lbsconstantly
• Performing CPR
• Providing moderate to maximumassistancewith patient transfers and bed mobility
• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
• Wearing PPE whenrequired

In New Jersey, the standard base pay range for a Full-Time role is $70,000 - $102,000 annually with an opportunity to earn more. Part-Time and FOX Flex (PRN) will be pro-rated.
