Skilled Nursing Facility (SNF) Physical Therapist

Responsibilities

Provide comprehensive therapy services to patients in a SNF setting, assessing and developing treatment plans, implementing therapy interventions, and ensuring patient progress. Maintain accurate documentation and collaborate with healthcare teams to optimize patient care.

Qualifications & Skills

Valid license as a Physical Therapist, relevant clinical experience, strong communication skills, and the ability to work independently. Knowledge of SNF protocols and patient-centered care is preferred.
