Qualifications

• Participation in mandatory hospital/system requirements and adherence to WellStar policies.

Responsibilities

• Implements the physical therapy treatment plan and progresses patients toward goals.
• Collaborates with physicians and staff to plan and evaluate treatment.
• Provides patient/family education, discharge instructions and referrals to resources.
• Documents evaluation, plan of care and treatment sessions per department guidelines.
