  • Collaborate with physicians, nurses, occupational therapists, and other rehabilitation team members.
  • Maintain accurate and timely documentation of evaluations, treatment sessions, progress, and outcomes.
  • Support discharge planning by assessing functional readiness and recommending continued therapy or equipment.

As the Physical Therapist, you will evaluate patients’ functional abilities, develop individualized treatment plans, and provide therapeutic interventions designed to improve mobility, strength, and independence. You will work closely with patients, families, and interdisciplinary care teams to support rehabilitation goals, monitor progress, and promote safe, effective recovery.

Essential Duties and Responsibilities
  • Evaluate patients’ mobility, strength, balance, range of motion, and overall functional abilities.

  • Develop and implement individualized treatment plans based on patient needs, diagnoses, and rehabilitation goals.

  • Provide therapeutic exercises, mobility training, gait training, and other physical therapy interventions.

  • Help patients improve strength, movement, independence, and overall functional performance.

  • Monitor patient progress and adjust treatment plans as clinically appropriate.

  • Educate patients and caregivers on exercises, mobility techniques, equipment use, and strategies for continued recovery.

  • Recommend and assist with the use of adaptive equipment, assistive devices, and mobility aids as needed.

  • Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the rehabilitation team.

  • Maintain accurate and timely documentation of evaluations, treatment sessions, progress, and outcomes.

  • Support discharge planning by assessing functional readiness and recommending continued therapy, equipment, or services.

  • Follow applicable clinical, safety, regulatory, and infection-control standards.

  • Promote a safe, supportive, and patient-centered rehabilitation environment.

  • Other duties as assigned

Qualifications
  • Active and unrestricted Physical Therapist (PT) license in the applicable state of practice.

  • Knowledge of physical therapy principles, rehabilitation techniques, and patient-centered care.

  • Strong clinical assessment, treatment planning, and documentation skills.

  • Effective communication and interpersonal skills with patients, families, and interdisciplinary care teams.

  • Ability to work independently while collaborating effectively within a rehabilitation or healthcare setting.

  • Ability to meet the physical demands of the role, including assisting patients with mobility and therapeutic activities.

  • Successful completion of a standard background check.

  • Successful completion of required pre-employment physical and tuberculosis (TB) screening.

  • Must successfully pass the required drug screening; a clear drug screen is mandatory for employment.
