Job summary

ABL Health Care is seeking a full-time Physical Therapist Assistant (PTA) to join our Home Health team in Vallejo. You will evaluate patients, develop individualized treatment plans, and work to improve mobility and overall quality of life with the interdisciplinary team.

Qualifications

• Current California PTA license required.
• Experience in Home Health care (1 year) preferred.

Responsibilities

• Provide physical therapy services within the patient's plan of care in coordination with the Interdisciplinary Team.
• Administer treatments, monitor progress, and report changes to the physician or director as needed.

Position Overview

We are seeking an experienced, self-driven Full Time Physical Therapist Assistant (PTA) to join our dedicated team and provide exceptional care throughout the region. As a PTA, you will evaluate patients’ physical health, develop individualized treatment plans, and work to improve their mobility, strength, and overall quality of life. You will collaborate closely with our Clinical Director and Clinical Staff, ensuring the highest standards of care while providing support both remotely and independently in the field.

Responsibilities

• Provide physical therapy services within the scope of care, following the patient’s individualized plan, in coordination with the Interdisciplinary Team.
• Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or Director of Nursing.
• Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
• Comply with all company policies and procedures to ensure high standards of care.

Requirements

• Current Physical Therapist Assistant (PTA) License to practice in California - Required
• Experience in Home Health care - 1 Year Required
• Current CA Driver’s License.
• Able to travel within a 15 - 25 mile radius on a regular basis.
