TheraCare is seeking a licensed Physical Therapist Assistant (PTA) for a full‑time Home Health role in Willow Brook, TX. You will provide one‑on‑one home-based physical therapy to adults and seniors, following a Plan of Care set by a supervising PT.

You’ll enjoy flexible daytime scheduling, autonomy in patient care, and the opportunity to improve mobility and safety in patients’ homes across Willow Brook and surrounding areas.

Qualifications

• Associate degree from an accredited PTA program.
• Active Texas PTA license.
• Home Health experience (1 year required).
• HomeCareHomeBase (HCHB) experience preferred.
• Current CPR certification.
• Strong communication, clinical judgment, and time-management skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide skilled home health PTA interventions under the supervision of a licensed PT.
• Implement and carry out Plans of Care developed by the supervising PT.
• Treat patients with orthopedic, neurological, cardiac, and post-surgical conditions.
• Administer therapeutic exercises, gait, balance, transfer, and functional mobility activities.
• Apply manual therapy techniques and non-complex modalities.
• Educate patients and caregivers on home exercise programs and safety.
• Monitor patient progress and report changes to the supervising PT.
• Complete accurate electronic documentation per Medicare home health regulations.
• Follow infection control standards and home health safety protocols.
• Participate in QAPI and agency performance improvement initiatives.

• Apply manual therapy techniques and non‑complex modalities including heat, cold, electrical stimulation, and other physician-ordered treatments
• Educate patients and caregivers on home exercise programs (HEP), fall prevention, and safe mobility strategies
• Monitor patient response to treatment and promptly report changes in condition to the supervising Physical Therapist
• Complete accurate and timely electronic documentation in compliance with Medicare home health regulations
• Follow infection control standards and home health safety protocols
• Participate in QAPI and agency performance improvement initiatives
