Job summary

Transition Home Healthcare in Houston, TX is seeking a dedicated Physical Therapist Assistant (PTA) to support patients in their homes and communities.

The PTA will assist in rehabilitation, implement care plans, coordinate with the home health team, educate patients and families, and prepare complete clinical notes, while communicating effectively with the care team.

Shifts available: PRN & Full Time. Home health experience preferred; Texas PTA license required; competitive benefits apply.

Qualifications

• Texas PTA license is required.
• Home health experience preferred.
• Knowledge of home health regulations and general healthcare compliance.
• Experience with HCHB EMR preferred.
• Able to handle physical workload.
• Good driving record.
• Excellent communication and people skills to interact with patients, families, and healthcare team members.

Responsibilities
