You will develop individualized treatment plans, implement therapeutic exercises, and collaborate with a multidisciplinary team to ensure coordinated, patient-centered care in the home setting.

Qualifications

• Current Physical Therapist Assistant license in California.
• CA Fingerprint Clearance Card.
• BLS (CPR) / ACLS certification.
• Recent TB test and flu shot within the past year.
• Immunization records including Covid-19, Hep B, MMR, TDaP, Varicella.
• Valid driver’s license with auto-insurance.

Responsibilities

• Deliver skilled physical therapy interventions, including exercises and stretches, to improve mobility and function.
• Create and execute individualized treatment plans tailored to each patient's needs and goals.
• Collaborate with a multidisciplinary team to ensure holistic and coordinated patient care.

Education

• Recent Negative TB test result and Flu Shot within the past year.
• Comprehensive Immunization/Vaccination Records, including Covid19, Hep B, MMR, TDaP, and Varicella.
• Valid Driver's License along with auto-insurance.
