Ascension at Home is seeking a Home Health Physical Therapist Assistant (PTA) to work in patients' homes, delivering one-on-one therapeutic services to improve strength and independence. You will collaborate with a compassionate care team to implement individualized plans and empower patients to perform daily living activities.

The role requires an accredited PTA degree, current state licensure, CPR, and at least one year of home health experience, with flexible scheduling and strong support for

Qualifications

• Must have Bachelor or Associate Degree from an accredited assistant school of Physical Therapy.
• Currently licensed in state of practice.
• CPR certification required; TB screen and Hepatitis consent/declination on file.
• Minimum one year experience in a home health setting.

Responsibilities

• Provide one-on-one in-home PTA services to patients.
• Assist patients to regain strength and independence for daily living activities.
• Collaborate with care team to implement treatment plans and document progress.
