Job summary

Adaptive Home Health, LLC is seeking a Physical Therapist Assistant (PTA) to deliver skilled PTA visits in patients' homes across Texas under a supervising PT. This field-based, full-time role emphasizes gait and balance, transfers, and safe mobility with measurable progress within a productivity-based compensation model.

You will coordinate with the care team, educate patients and caregivers, and travel daily to assigned territories, ensuring high-quality home health care and patient outcomes.

Qualifications

• Active Texas PTA license in good standing.
• Strong clinical judgment and comfort working independently in the field (within PTA scope and under PT supervision).
• Comfortable escalating changes in condition to PT/RN/MD and documenting rationale clearly.
• Strong communication and documentation.
• Reliable follow-through on treatment plans, visit cadence, and patient goals.
• Current driver’s license and comfort with daily travel across assigned territory.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health PTA scope: mobility, gait/balance, transfers, safety.

Physical Therapist Assistant (PTA) - Home Health

• Schedule: Full-time Monday–Friday + occasional weekends (as needed)

• Location: Field Based

• Compensation $58 per Point - 30 Points per Week

Requirements

Must-haves

• Active Texas PTA license (in good standing)

• Strong clinical judgment and comfort working independently in the field (within PTA scope and under PT supervision)

• Comfortable escalating changes in condition to PT/RN/MD and documenting rationale clearly

• Prior home health experience

• Homecare Homebase (HCHB) experience (or similar home health EMR)

• Deeper clinical experience in high-signal ortho/neuro populations
