Job summary

Phoenix Home Care & Hospice in Clinton, MO is seeking a Physical Therapist Assistant for home health. The PTA will work under a licensed Physical Therapist to deliver patient-centered care and help patients achieve mobility goals in a home setting.

Applicants should have MO PTA certification; home health experience is a plus but not required. The role includes charting, multidisciplinary collaboration, and adherence to agency standards, with a focus on compassionate patient care.

Qualifications

• PTA certification for MO is required or preferred.
• Home Health experience is a plus but not required.
• Must pass drug test, background check, and physical.
• Must be able to collaborate with a multidisciplinary care team.

Responsibilities

• Provide PTA services to patients in home health under licensed PT supervision.
• Communicate and participate in case conferences to ensure continuity of care.
• Perform treatments and procedures per agency protocols with sound judgement.
• Maintain compliance with agency Code of Ethics and applicable regulations.
• Comply with all policies, procedures, and payer requirements.
• Follow safety, infection control, and environmental guidelines.
• Perform other duties as assigned by Supervisor.

Requirements

• Physical Therapist Assistant Certification for the state of MO
• Home Health experience preferred, not required.
• Ability to Pass Drug Test, Background Check, and Physical
• Multi-disciplinary collaboration for patient care

Responsibilities

• Provides PT assigned services to patients in the home setting in accordance with Physician plan of care and under the supervision of a licensed PT.
• Participates in communication and case conference activities to promote maximum continuity and quality of patient care services.
• Exhibits sound judgement and technical skills necessary to perform assigned treatment and procedure according to established agency protocols.
• Maintains compliance with the Agency Code of Ethics.
• Demonstrates ongoing compliance with agency standards, policies, procedures, state and Federal regulations, state practice acts for licenses personnel, payer requirements.
• Practices safety, environmental, and/or infection control measures.
• Performs miscellaneous job-related duties as assigned by Supervisor.
