Job summary

HealthPro Heritage in Sayville, NY is seeking a Licensed Physical Therapist Assistant for a PRN role in a senior living community. The position offers M-F therapy with no evenings or weekends, and a competitive benefits package.

The PTA will assist in implementing treatment plans, document patient progress, and collaborate with Physical Therapists and care teams to ensure high-quality care and positive outcomes.

Qualifications

• Associate’s degree in Physical Therapist Assisting from an accredited program.
• Valid state licensure as a Physical Therapist Assistant, or license eligible
• Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills.

Responsibilities

• Assist in implementing treatment plans designed by Physical Therapists.
• Provide direct patient care including therapeutic exercises, manual therapy, and modality applications.
• Monitor and document patient progress, report observations to the Physical Therapist, and educate patients and families on treatment protocols, exercises, and self-care techniques.
• Accurately document patient treatments, progress, and responses in compliance with facility policies and regulatory standards.

Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems, with a proven ability to work collaboratively in a multidisciplinary team.
