• Conduct thorough clinical assessments and record findings in electronic health records (EHR), supporting effective discharge planning and ongoing treatment adjustments.
• Support patient care activities across multiple settings including outpatient clinics, inpatient units, skilled nursing facilities, home health visits, and acute care environments.
• Collaborate with interdisciplinary teams to develop comprehensive care plans that address individual patient needs and goals.

Inpatient experience with skilled nursing facilities or acute care settings is highly desirable.

Knowledge of discharge planning processes to facilitate smooth transitions for patients returning home or to other care environments.

Outpatient clinic experience in orthopedics, geriatrics, pediatrics, or general physiotherapy is preferred.
