Job summary

Elara Caring in Illinois is seeking a Physical Therapist Assistant to join our home-visits team. You will deliver direct therapy under supervision, following the Plan of Care and reporting changes to the supervising PT.

You may travel within the branch area to provide patient care in homes. Applicants must have an accredited PTA program, an unrestricted PTA license for Illinois, and be willing to travel up to 50% with the ability to lift 50–100 lbs.

Qualifications

• Graduate of an accredited physical therapy assistant program approved by the American Therapy Association.
• 6+ months experience as a Physical Therapist Assistant is preferred.

Responsibilities

• Performs direct physical therapy services under the supervision of a Physical Therapist to ensure quality and continuity of care.
• Follows Plan of Care as established by the Physical Therapist and reports changes as required.
• Instructs patients and/or families in the safe use of prosthetic, orthotic and assistive devices and home therapy programs.
• Willingness to travel within the branch/office coverage area and work in patients' homes.
