FOREST GROVE POST ACUTE seeks a Staff Physical Therapist Assistant to support Physical Therapists in delivering direct patient care and documentation. The role involves following treatment plans, recording progress, and helping maintain a clean, functional therapy area.

You will collaborate with nursing and rehab staff, participate in conferences, and assist with family education and discharge planning as needed.

Responsabilités

• Treat patients as directed by Physical Therapist.
• Record daily treatment notes and weekly progress notes per PT Board.
• Assist in maintaining department.
• Participate in Patient Care and Rehabilitation Conferences, as needed.
• Assist with cleaning and maintenance of treatment area.
• Treat patients per the physician treatment plan.
• Assist nursing department with training of Restorative Aides.
• Communicate with supervisor and other health team members regarding patient progress, problem and plans.
• Participate in in-services training program for other staff in the facility.
• Record treatment changes per policy and procedures.
• Instruct patient’s families or nursing staff in maintenance program and caregiver training in preparation for discharge from therapy services.
• Report any problems with department equipment so that it is maintained in good working order.
• Ability to relate positively, effectively, and appropriately with patients/residents, families, staff and professional colleagues

General Purpose

The Staff Physical Therapist Assistant assists the Staff Therapist with patient related activities and direct patient care.
