One Home Health is seeking a compassionate Physical Therapist Assistant (PTA) to join our home health team serving Porter County, Indiana, including Valparaiso, Portage, Chesterton, and surrounding communities.

Pay ranges from $45 to $55 per hour with Flexible scheduling and options for Full-Time, Part-Time, or PRN positions. Candidates should be able to travel across Porter County and enjoy one-on-one patient care in home settings.

Formación

• Active Indiana PTA license.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.

Responsabilidades

• Provide physical therapy treatments under the supervision of a licensed Physical Therapist.
• Help patients improve mobility, strength, balance and independence at home.
• Educate patients and caregivers on exercises, safety and fall prevention.

Home Health Physical Therapist Assistant (PTA) | Porter County, IN

One Home Health is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our growing home health team serving Porter County, Indiana, including Valparaiso, Portage, Chesterton, and surrounding communities.

Pay: $45-$55 per hour
Schedule: Full-Time, Part-Time, or PRN

What You'll Do

• Provide physical therapy treatments under the supervision of a licensed Physical Therapist.

• Help patients improve mobility, strength, balance, and independence in their homes.

• Educate patients and caregivers on exercises, safety, and fall prevention.

• Complete documentation and communicate patient progress with the care team.

Qualifications

• Active Indiana PTA license.

• Current CPR/BLS certification.

• Valid driver's license, reliable transportation, and auto insurance.

• Strong communication and documentation skills.

• Ability to travel throughout Porter County.

Preferred

• Home health experience.

• Knowledge of Medicare documentation requirements.

• Experience with Axxess software.
