Therapy Partner Solutions is seeking a Licensed Physical Therapist Assistant to support our Home Health team in Gwinnett County, GA. The role offers Part-time and PRN options with a cohesive, multidisciplinary team.

Qualifications

• Graduate (current or pending) of an accredited physical therapy assistant program.
• Active or pending state physical therapist assistant license.
• Comfort working independently in the home setting.

Responsibilities

• Provide PTA services to patients in a home health setting with a diverse caseload.
• Collaborate with PTs, OTs, nurses and other clinicians for integrated care.
• Document patient progress and care plans in the chart.
• Support patient-centered care with emphasis on outcomes and safety.
