Job summary

Haven Home Health & Hospice in Missouri is seeking a Physical Therapist Assistant to join our client‑centered team. You will provide skilled PTA services under supervision, helping clients stay safe, independent, and active in their homes.

The ideal candidate holds a Missouri PTA license, has at least one year of recent PTA experience, and demonstrates strong communication, judgment, and comfort with electronic documentation. Weekend/holiday availability is a plus.

Qualifications

• Missouri PTA license in good standing is required.
• At least one year of PTA experience within the last three years.
• Strong communication, organizational and collaboration skills.

Responsibilities

• Provide skilled physical therapy services under the supervision of a Physical Therapist.
• Implement individualized plans of care based on physician orders and client needs.
• Monitor client progress and communicate changes with the supervising PT and care team.
• Provide preventive and rehabilitative therapy interventions.
• Educate clients and families on self-management, safety, and physical therapy needs.
• Maintain accurate and timely clinical documentation.
• Collaborate with other Haven clinicians for coordinated, client-centered care.
• Participate in department meetings, in-services, outcomes, and performance improvement initiatives.
• Uphold Haven policies, professional standards, and applicable regulatory requirements.

As a PTA at Haven, you will work alongside a collaborative team of therapists and clinicians to provide skilled physical therapy services that help clients remain safe, independent, and active in their own homes. You will build relationships with clients and families while helping them work toward the goals outlined in their individualized plan of care.

What We’re Looking For

• Current Missouri PTA license in good standing

• Comfortable with technology and electronic documentation
• Dependable transportation and a current driver’s license
