Job summary

The Lakes Home Care, Inc. is seeking a dedicated Physical Therapist Assistant to support licensed therapists in delivering in-home rehabilitation. You will help patients regain movement, manage pain, and perform therapeutic exercises in a home setting.

Responsibilities include implementing treatment plans, documenting progress, educating patients and families, and collaborating with healthcare professionals to ensure comprehensive care.

Qualifications

• Experience in patient care, particularly in geriatrics, orthopedics, or hospital settings, is preferred or home care
• Knowledge of physiology and occupational health practices
• Familiarity with inpatient therapy procedures

Responsibilities

• Provide assistance to Physical Therapists in developing and implementing treatment plans
• Work with patients to help them regain movement and manage pain
• Monitor and document patient progress during therapy sessions
• Educate patients and their families on at-home exercises and care routines
• Collaborate with healthcare professionals to ensure comprehensive patient care
