Responsibilities

• Deliver skilled treatment under a PT-authored plan of care in patients’ homes — therapeutic exercise, mobility and gait training, manual therapy, neuromuscular re-education, and trigger-point techniques.
• Educate patients and caregivers on home exercises, mobility strategies, and safety.
• Document each visit accurately and communicate progress to the supervising PT.
• Keep every home visit safe and professional.
• Treat a range of cases — post-accident, post-surgical, and chronic-condition patients.
