Job summary

Westfield Rehabilitation and Nursing Center in Westfield, MA is seeking a PRN Physical Therapist Assistant (PTA) to provide compassionate rehabilitation services under the supervision of a Licensed Physical Therapist. You will join a team known for patient-centered care and a collaborative environment.

Responsibilities include assisting with treatment plans, documenting progress, educating residents and families, and contributing to a respectful, confidential setting that supports residents'

Qualifications

• Must have graduated from an accredited PTA program.
• Must hold a valid PTA license and registration in the state of practice.
• Experience with long-term care and/or geriatric care preferred, but not required.
• Experience with Case Mix Index and PPS scheduling preferred, but not required.
• Strong communication and teamwork skills.

Responsibilities

• Assist the Physical Therapist in providing direct patient care to residents requiring rehabilitation services.
• Implement treatment programs established by the Physical Therapist according to the patient's capacity and tolerance under the direction of the Physician.
• Monitor and document residents' responses to treatment and report progress or concerns to the supervising Physical Therapist.
• Help patients improve mobility, strength, balance, and functional independence.
• Educate residents and families on therapy techniques and safety procedures as directed by the Physical Therapist.
• Complete required documentation including daily treatment notes and other assigned records in a timely and accurate manner.
• Collaborate with interdisciplinary team members to provide patient-centered care.
• Perform all other duties assigned by the Director of Rehabilitation.
• Maintain compliance with confidentiality policies, HIPAA regulations, and Federal Residents' Rights.
