Job summary

Phoenix Home Care in Springfield, MO is seeking a licensed Physical Therapist Assistant to deliver home health services under the supervision of a PT. You will support treatment plans, document charting during visits with the help of AI-powered tools, and help patients regain mobility and independence.

Responsibilities

• Provide PTA-assigned services to home health patients under licensed PT supervision.
• Participate in communications and case conferences to ensure continuity of care.
• Perform treatments per agency protocols and demonstrate sound judgement.
• Maintain compliance with Agency Code of Ethics and state/federal regulations.
• Adhere to safety, infection control, and environmental standards during visits.
• Contribute to maintaining high quality patient care and documentation.
• Perform miscellaneous duties as assigned by Supervisor.

Responsibilities

• Provides PT assigned services to patients in the home setting in accordance with Physician plan of care and under the supervision of a licensed PT.
• Participates in communication and case conference activities to promote maximum continuity and quality of patient care services.
• Exhibits sound judgement and technical skills necessary to perform assigned treatment and procedure according to established agency protocols.
• Maintains compliance with the Agency Code of Ethics.
• Demonstrates ongoing compliance with agency standards, policies, procedures, state and Federal regulations, state practice acts for licences personnel, payer requirements.
• Practices safety, environmental, and/or infection control measures.
• Performs miscellaneous job-related duties as assigned by Supervisor.

Requirements

• Physical Therapist Assistant Certification for the state of MO
• Home Health experience preferred, not required
• Ability to Pass Drug Test, Background Check, and Physical
