Job summary

NHC HomeCare Low Country in North Charleston, SC, is seeking a Physical Therapist Assistant to deliver skilled PT treatments under the supervision of a Physical Therapist. You will help implement treatment plans, monitor patient responses, and contribute to achieving therapy goals.

The role requires an accredited PTA program degree, state licensure, and a commitment to accurate documentation and teamwork in a patient‑centered home care setting.

Qualifications

• An associate degree from PTA program accredited by APTA is required.
• Current license as a Physical Therapist Assistant in the state of practice.
• Active membership in state and national PTA associations preferred.

Responsibilities

• Provides physical therapy services under the direction of a licensed PT.
• Assists in evaluating progress toward goals and documents findings.
• Maintains daily treatment records and progress notes as directed by the therapist.
• Observes and reports patient responses and conditions to the PT.

Physical Therapist Assistant - NHC HomeCare Low Country

The Physical Therapist Assistant is a skilled technical provider of physical therapy treatments and related duties working under the direction of the Physical Therapist.

Qualifications:

• Must have an Associate’s Degree from a recognized Physical Therapist Assistant Program accredited by the American Physical Therapy Association.
• Must be licensed in the state in which the assistant will be practicing.
• Active member of state and national Physical Therapy Associations preferred.

Job Highlights:

• Provides physical therapy services as directed by the physical therapist and ordered by the physician according to policies and procedures and state/federal regulation.
• Assists the physical therapist with evaluation of progress toward goals.
• Observes records and reports to the therapist the conditions, reactions and responses related to assigned duties.
• Maintains daily records of treatments. Documents progress notes regarding patients as directed by the therapist.
