Job summary

Ventura Services Florida is seeking a dedicated Physical Therapist Assistant to support licensed physical therapists in a skilled nursing facility in Florida. You will assist with treatment plans, document services, and monitor residents’ responses under PT supervision.

The role requires a Florida license and registration, and preferred candidates have 2 years of experience, including geriatrics. Excellent communication with staff and families is essential.

Qualifications

• Graduate of approved Physical Therapy program (Bachelor's, Master's or DPT).
• Florida State License and current Florida Registration Certificate.
• Preferred: 2 years of experience, at least one in geriatrics.

Responsibilities

• Performs treatment sessions as directed by PT.
• Documents and maintains service-related records.
• Assists PT in developing and implementing treatment plans.
• Implements and modifies treatment plans under PT supervision.
• Monitors resident response to treatment and reports observations.
• Maintains daily statistics in Net Health.
• Recommends termination of services as directed.
• Provides in-service education to nursing staff and family members.
• Communicates with other therapists and departments regarding discharge plans and changes.
• Aware of continuing education needs as per state policy.
• Attends department and interdisciplinary meetings.

3. Assists PT in developing treatment plans & techniques to implement plan.
4. Implements & modifies treatment plans under PT supervision.
5. Monitors Resident's response to treatment & reports oberservations to supervising therapist.
6. Maintains daily statistics records in Net Health.
7. Recommends to supervisor termination of services.
8. Provide in-service education to nursing staff & family members as needed.
9. Communicates effectively with other therapist & departments re: common issues (discharge plans & significant changes).
10. Aware of state requirement Continuing Education need as per State Policy.
11. Attends all department & interdisciplinary meetings as scheduled.
12. Complies with HIPAA guidelines.
