Job summary

Tender Touch Rehab Services is seeking a compassionate Physical Therapist Assistant to support high-quality rehab services for seniors in skilled nursing and long-term care settings.

In this on-site role, you’ll carry out treatment plans designed by Physical Therapists, providing therapeutic exercises, gait and balance training, and functional mobility interventions that enhance residents’ safety, independence, and quality of life.

Qualifications

• Provide compassionate patient care under supervision of Physical Therapists.
• Collaborate with interdisciplinary care teams to support resident needs.
• Maintain timely and accurate clinical notes in EMRs.

Responsibilities

• Carry out treatment plans designed by Physical Therapists.
• Provide therapeutic exercises, gait and balance training, and functional mobility interventions.
• Document progress in EMRs and report to supervising PT.
• Partner with an interdisciplinary care team to optimize resident outcomes.
