Job summary

Blue Stone Therapy is seeking a Physical Therapist Assistant (PTA) to join our Laurens, IA team, providing rehabilitation services in a SNF/post-acute setting under PT supervision.

The role focuses on mobility, strength, balance, and functional independence for adults and geriatrics; a $5,000 sign-on bonus is available for eligible full-time candidates. Requires an accredited PTA program, active license, and strong communication and organizational skills.

Qualifications

• Graduate of an accredited Physical Therapy Assistant program.
• Current Physical Therapy Assistant license (or eligibility).
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.
• Strong communication, organization, and clinical reasoning skills.

Responsibilities

• Provide skilled physical therapy treatment in accordance with established plans of care.
• Implement individualized treatment programs focused on functional outcomes.
• Treat adult and geriatric patients in SNF and rehabilitation settings.
• Monitor patient response and communicate progress with supervising PT.
• Maintain timely documentation to support quality care and reimbursement standards.

Physical Therapist Assistant (PTA) - Laurens, IA

Blue Stone Therapy is seeking a Physical Therapist Assistant (PTA) to provide rehabilitation services to adult and geriatric patients at Laurens Care Center in Laurens, IA.

This role focuses on delivering skilled physical therapy services to adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Physical Therapy Assistant works under the supervision of a licensed Physical Therapist to implement treatment plans designed to improve mobility, strength, balance, endurance, pain management, and overall functional independence.

$5,000 sign-on bonus available for eligible full-time candidates.

What You’ll Do

• Provide skilled physical therapy treatment in accordance with established plans of care
• Implement individualized treatment programs focused on functional and measurable outcomes
• Treat adult and geriatric patients in a skilled nursing facility (SNF) setting and rehabilitation settings
• Monitor patient response to treatment and communicate progress with the supervising Physical Therapist

• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes
