Phoenix Home Care & Hospice is seeking a Part Time, Weekday Visits Physical Therapist Assistant to deliver in-home care to patients in Joplin, MO and surrounding areas. You will work under a licensed PT, helping patients reach mobility goals and maintain independence.

Qualifications

• PTA certification valid in Missouri.
• Home Health experience is preferred but not required.
• Ability to pass drug test, background check, and physical.
• Multidisciplinary collaboration for patient care.

Responsibilities

• Provide PTA services to patients in home setting under licensed PT supervision.
• Participate in communications and case conferences to ensure continuity of care.
• Apply clinical judgement and skills per agency protocols.
• Maintain compliance with Agency Code of Ethics.
• Adhere to agency standards, policies, and state/federal regulations.
• Follow safety, infection control, and environmental measures.
• Perform other duties as assigned by Supervisor.

Responsibilities

• Provides PT assigned services to patients in the home setting in accordance with Physician plan of care and under the supervision of a licensed PT.
• Participates in communication and case conference activities to promote maximum continuity and quality of patient care services.
• Exhibits sound judgement and technical skills necessary to perform assigned treatment and procedure according to established agency protocols.
• Maintains compliance with the Agency Code of Ethics.
• Demonstrates ongoing compliance with agency standards, policies, procedures, state and Federal regulations, state practice acts for licenses personnel, payer requirements.
• Practices safety, environmental, and/or infection control measures.
• Performs miscellaneous job-related duties as assigned by Supervisor.

Requirements
