Performance Optimal Health is seeking an Outpatient Physical Therapist Assistant to support clinicians in delivering personalized rehabilitation plans to clients in our Connecticut area. You will follow established treatment plans, educate patients and families, and ensure thorough daily documentation while contributing to high-quality clinical care.

We welcome recent PTA graduates with an accredited program and 0–4 years of experience.

Qualificações

• Must have valid PTA license in the United States.
• 0–4 years experience; open to recent graduates.
• Associates degree from PTA program.

Responsabilidades

• Instructs patients and caregivers in PT techniques under supervision.
• Implements individualized treatment plans established by the Physical Therapist.
• Maintains daily chart documentation of interventions and services.
• Contributes to clinical outcomes through assessments and continuing education.
• Collaborates with the Optimal Health team to deliver programs tailored to each patient.

• You are an integral part of the Client Experience and work side by side with patients throughout their Optimal Health journey
• Ensures clinical excellence through clinic outcome assessments, mentorship, and continuing education
• Top-notch, empathic clinical care to our patients
• Implements patient's individualized treatment plan as established by the primary Physical Therapist
• Instructs patients, families, and other caregivers in the skills and techniques of the physical therapy treatment program under the supervision of the Physical Therapist
• Work with other members of the Optimal Health Team to implement programs consistent with the needs and capabilities of each patient
• Provides daily and timely patient chart documentation of interventions and services provided in the acute care setting

• Performs other job-related duties as assigned.

• 0-4 years’ experience and must possess a valid license (Open to recent graduates).
• Associates degree, accredited by an accredited PTA Program
• Ability to work in a fast-paced team environment.
• Self-direction with a strong sense of team commitment
