Job summary

FOX Rehabilitation in Annapolis, MD, seeks licensed Physical Therapists to deliver one-on-one care to older adults in private residences or Senior Living Communities, using our Geriatric House Calls model. You’ll develop individualized plans, deliver evidence-based treatments, and collaborate with an interdisciplinary team to improve mobility and independent living.

• Active PT license in the state(s) of practice or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Current CPR certification.
• Basic computer and EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain


• Degree from an accredited Physical Therapy program


• Basic computer and EMR proficiency


• Current CPR certification

Physical Requirements

• Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly


• Performing CPR


• Providing moderate to maximum assistance with patient transfers and bed mobility


• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities


• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model


• Wearing PPE when required

In Maryland, the standard base pay range for a Full-Time role is $70,000 - $110,000 annually with an opportunity to earn more. Part-Time and FOX Flex (PRN) will be pro-rated.
