Unity Hospital in Rochester, NY, is seeking a Physical Therapist – Rehab for an inpatient acute care unit on a day shift. Per diem schedule includes 8-hour shifts with weekend rotation. Responsible for treatment planning, discharge planning, and patient-family education.

Leadership duties include staff development and in-service training. Requirements include a Master’s in Physical Therapy (or comparable) and NY license; licensure within 90 days if not held.

Qualifications

SUMMARYAs a Physical Therapist – Rehab, your ability to design, plan, and implement personalized physical therapy intervention greatly enhances patient function and overall quality of life.STATUS: Per DiemLOCATION: Unity Hospital - 1555 Long Pond Rd, Rochester, NY 14626DEPARTMENT: Inpatient Acute CareSCHEDULE: Day shift, 8-hr. shifts; 8:00 am - 4:30 pm; weekend rotation requires at least one (1) weekend per monthRESPONSIBILITIESTreatment & Planning. Meet patient goals and needs by providing quality, evidence-based care; assess, develop, and implement an individualized treatment plan; help the patient accomplish the treatment plan and accept therapeutic devices by administering manual exercises; instruct, encourage, and assist patients in performing physical activities, such as non-manual exercises, ambulatory functional activities, and daily-living activitiesDischarge Management. Assure continuation of therapeutic plan following discharge by designing home exercise programs; instruct patients, families and caregivers in home exercise programs; recommend and provide assistive equipment; recommend outpatient or home health follow-up programsLeadership.
