Job summary

Essentia Health in Duluth, MN is seeking a licensed Physical Therapist to evaluate, plan, treat, and implement care for patients in accordance with APTA standards and Essentia Health policies.

This role supports a full caseload in inpatient and outpatient settings, collaborates with a multidisciplinary team, participates in discharge planning, and advances clinical skills through education and leadership opportunities.

Qualifications

• Bachelor's, Master's, or Doctorate in physical therapy from an accredited program.
• Current license in the state performing services.

Responsibilities

• Provide therapy evaluation, develop individualized treatment plans, implement evidence-based interventions, and discharge planning for a full patient caseload.
• Provide all ancillary/administrative requirements for a full caseload of patients, including documentation, billing, scheduling management, and other administrative duties.
• Demonstrates communication, professionalism and supervision of support staff.
• Collaborate with a multidisciplinary care team to ensure optimal clinical and financial outcomes.
• Work closely with Essentia Health leadership and coordinate with referring physicians to ensure cohesive patient care.
• Comply with organizational policies and code of ethics.
• Work across departments when needed to meet patient care demands.
• May serve as a clinical instructor and participate in department or committee activities.

Job Description:

Evaluates, plans, treats and implements care for patients in accordance with professional standards of the American Physical Therapy Association and policies and procedures of Essentia Health.

Education Qualifications:

• Bachelor's Degree,
• Master's Degree,
• or Doctorate Degrees from a physical therapy accredited program

Key Responsibilities:

• Provide therapy evaluation, develop individualized treatment plans, implement evidence-based interventions, provide patient/caregiver education, and discharge planning for a full patient caseload.
• Provide all ancillary/administrative requirements for a full caseload of patients, including documentation, billing, scheduling management, and other administrative duties.
• Demonstrates appropriate communication, professionalism and supervision of support staff (licensed assistants, aides, volunteers).
• Collaborate with a multidisciplinary care team to ensure optimal clinical and financial outcomes.
• Work closely with Essentia Health leadership and coordinate with referring physicians to ensure cohesive patient care.
• Comply with organizational policies and procedures and the code of conduct, meet professional organization core values, code of ethics, &/or scope of practice.
• Work where the patient need is highest (including flexing to other departments) when home department schedule allows.
• May serve as a clinical instructor, participate in department or therapy discipline committees, and complete credential/certification that would benefit patient care.
• Inpatient Acute Care: Coordinate and deliver durable medical equipment to patients; collaborate with a multidisciplinary team to support smooth transitions of care.

Schedule:

Six shifts per pay period (2 weeks). Flexible start time between 7:30a - 9:00a. One weekend per month rotation.

• Compensation Range: $38.22 - $57.33.
