Job summary

Northshore Home Health Care in Sioux Falls is seeking a licensed Physical Therapist to coordinate and deliver patient-centered rehabilitation services. You will assess patients, develop individualized treatment plans, and educate patients and caregivers on exercises and home programs.

The role includes supervising clinical staff, documenting evaluations and progress, and collaborating with an interdisciplinary team.

Qualifications
