Providence at Home with Compassus is seeking a Physical Therapist to deliver home-based therapy evaluations, treatment planning, and discharge planning in patients' homes within California. You will work with a multidisciplinary team to communicate client needs, coordinate care, and supervise aides as assigned.
The role requires a Bachelor's in Physical Therapy, active California license, BLS and American Heart Association credentials, and the ability to lift up to 50 pounds.

Position Summary This position is responsible for therapy evaluations, treatment planning, implementation and discharge planning for clients in the home setting in accordance with established facility/agency policies while maintaining the highest degree of quality client care. As a member of the multi-disciplinary team, the therapist assumes responsibility for communication of client related issues for appropriate team members or other facility/agency staff and participates in the collection and evaluation of data necessary to assess the appropriateness of care at home. The Physical Therapist also assumes responsibility for those clients being treated by home health aides and may be assigned to orient staff and students. The Physical Therapist works with the clients and family in the home setting without direct supervision.
Bachelors Degree in Physical Therapy from an institution approved by the state board.
Preferred - 1 year Therapy with adult physical disabilities.
The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.

Compensation range: $59.40 - $73.37
additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
