Physical Therapist - $5,000 Sign-On Bonus!

Help older adults stay safe, independent, and at home, and get a $5,000 sign on BONUS with eligibility to earn additional quarterly bonuses!

This is not traditional physical therapy. It is the opportunity to use your clinical judgment where it matters most: helping older adults maintain mobility and continue to live in the homes know and love.

The Opportunity

Join an interdisciplinary PACE team built around access, outcomes, and meeting needs, not visit restrictions, productivity, or limited access to basic DME needs. This role is based at the PACE center and includes home visits, allowing you to assess and treat participants in both a collaborative clinical setting and the environments where they live.

Why Exceptional PTs Choose PACE
  • Clinical decisions driven by need: Recommend the right intervention at the right time, with care planning centered on participant goals and safety rather than traditional insurance visit constraints.
  • Real continuity of care: Build trusted relationships and follow participants over time, adjusting the plan as health, function, environment, and priorities change.
  • True interdisciplinary practice: Work side by side with primary care, nursing, occupational therapy, social work, nutrition, transportation, home care, and other partners.
  • Impact beyond the treatment space: Influence mobility, fall prevention, caregiver support, equipment, home safety, and successful transitions across settings.
  • Meaningful complexity: Apply advanced clinical reasoning to medically and socially complex situations where your expertise can prevent decline, hospitalization, or facility placement.
What You’ll Do
  • Make a meaningful impact by completing PT Assessments and treatment by using evidence based skilled treatments to attain participant highest functional mobility and quality of life.
  • Achieve Participants goals by improving mobility, strength, balance, and independence through therapeutic exercise, functional activities, gait training, and physical modalities.
  • Use your clinical expertise to create individualized plans that prioritize safety, independence, meaningful activity, and the participant’s goals.
  • Document efficiently and effectively to track outcomes and continuously improve how therapy reaches participants.
  • Recommend durable medical equipment, orthotics, and prosthetics or other adaptive solutions that support safety, comfort, and daily function.
  • Develop and facilitate transitional restorative and wellness programs that promote long‑term function and quality of life.
  • Build trusted relationships with Participants, caregivers, families, supportive staff, and healthcare partners.
  • Provide clinical guidance and supervision to Physical Therapist Assistants.
  • Enjoy variety in your work by delivering therapy across the PACE Center, Participants’ homes, skilled nursing facilities, and hospitals.
Qualifications
  • Must hold a current PT license within the state you are practicing in
  • Ability to work across care settings and travel locally as the role requires.
  • Experience with older adults, home-based care, geriatrics, complex care, or interdisciplinary practice is valued; exceptional clinicians from all settings are encouraged to apply.
What You Can Expect
  • A mission‑driven environment where therapy is central to the model of care.
  • Protected collaboration time and direct access to an interdisciplinary team.
  • Support for thoughtful, proactive care—not productivity for productivity’s sake.
  • Opportunities to shape programs, improve access, and advance best practices in healthy aging.
  • A competitive compensation and benefits package aligned with the scope and value of the role.
  • 9 Paid Holidays
  • PTO starting at 3 weeks + 1 day per year (accrued from date of hire for full‑time employees)
  • Time spent during working hours for approved CEUs and license renewals and will be paid up to 3 days per year.
  • Reimbursement for approved CEUs and license renewals will be up to a maximum of $800 per calendar year.
  • Free Life and AD&D Insurance Plan
  • Health Savings and Flexible Spending Accounts
  • Short Term Disability Insurance
  • Group Voluntary Term Life Insurance for Employee, Spouses, and Dependents
  • Tuition Reimbursement and Paid Training Opportunities
  • Retirement Plan with company annual match
  • Mileage Reimbursement at annual IRS rate as applicable
